Greenbelt Kennels and the undersigned owner, or people having custody of the animal noted above, agree as follows:

The undersigned requests Greenbelt Kennels board the pet(s) and agrees to pay for each night of boarding and for all agreed upon services when the pet is picked up.

If any charges for boarding, grooming, training, medicine, or veterinary services are not paid and the owner does not claim the animal by the agreed date, the kennel is hereby authorized to sell or otherwise dispose of said animal(s) in its absolute discretion per Iowa Code 162.19 (the abandoned animals) which is clearly posted on our web site.

We agree your pet(s) are to be fed properly and regularly; to be housed in clean, safe quarters; and provided proper medication. All dogs are boarded by Greenbelt Kennels without liability on our part for loss or damage from disease, death, running away, theft, fire, injury to persons, other dogs, or property by said pet, or other unavoidable causes, due diligence and care having been exercised.

If my pet should become sick, ill or in need of medical attention while boarding, I hereby authorize Jean Brown or the Kennel Manager, to use their judgment, in my absence, to take my pet to my pet's veterinarian listed above or The Animal Emergency & Referral Center of Central IA for appropriate medical attention and treatment at the owner's expense.

Dogs must be up-to-date on the following vaccinations: Distemper, Parvo, Hepatitis, Corona, Bordatella, and Rabies. Vaccinations will be given at owner expense if last vaccination(s) date cannot be verified. Cats must be up-to-date on the following vaccinations: Feline Viral Rhinotracheitis, Calicivirus, Panleukopenia, Chlamydia, and Radies. Feline Leukemia is recommended but not mandatory.

If fleas are found on your pet, they will be given a flea bath. This will only kill fleas on your pet at that time. Every attempt will be made to notify you as soon as the fleas are discovered so you can have time to treat your home, yard, car, pet bedding, etc. A $10 charge will be added to your grooming invoice for the flea bath and treatment of grooming areas.

All pets delivered to the kennel or picked up from the kennel outside of regular posted hours will be charged a $20.00 after-hours-service-fee.

Greenbelt Kennels is not responsible for the loss of toys, leashes, food dishes, bedding, blankets, etc. left with your dog.
